Health & Wellbeing Manager
It is an exciting time to become the Health and Wellbeing Manager at Woking Borough Council. You will be responsible for managing the Social Prescribing Team and be part of the Social Prescribing movement in North West Surrey. Working in partnership with the newly formed Primary Care Networks as well as the North West Surrey Integrated Care Partnership, you will be responsible for the new Social Prescribing Link Workers and the existing team to empower people to take control of their health and wellbeing by focussing on what matters to them and take a holistic approach, connecting people to community groups and statutory services for practical and emotional support. Referrals may be made for any one of a number of reasons, including inactive lifestyles, loneliness and isolation, anxiety, financial concerns, poor nutrition and those struggling with daily routine and staying independent.
You will work closely with PCN’s, GPs and neighbouring District and Boroughs to ensure a consistent approach is achieved across NW Surrey and monitor the performance of the team and ensure they are enabling residents to identify their needs and requirements and link them to community groups that can help them to become more independent and lead a more fulfilling lifestyle. You will also set up and maintain new referral routes for Social Prescribing in addition to the PCN’s.
You will also be help organise a full Living Well programme of activities throughout the year being part of the holistic Independent Living Team. These will predominately include social activities and events to engage residence that are aged 50+.
You must be organised, able to work independently and manage your time well to ensure you are able to meet changing workloads. You will need to be confident in making decisions, confident in multi-agency meetings and managing staff.
You will need to be practical and an effective communicator who can think on their feet. You will need to have good collaborative working skills to be successful in this role as you will be working with a wide range of partners; being able to communicate at different levels is key.
To be successful in this role you will need to have experience of working face to face with customers and / or service users. You will have experience of working in either a local government, health or voluntary sector organisation. Knowledge of the communities to be served by the post applied for, as well as services available locally is desirable but not essential.
